  •  What is a Care Manager?
A care manager is a professional who specializes in assisting older or disabled adults and their families in meeting their short or long range needs.
  • How our care management services can benefit you.
We can develop an individualized plan which provides for immediate support and long range needs.  This is accomplished by locating and arranging for services, screening and assisting in the hiring of providers for home care, medical, financial and legal services, home maintenance, meals, transportation, pet care and other services.
 
We can accompany clients to medial appointments and coordinate services, monitor health and other care providers in addition to periodically re-assessing the client and his or her needs.  We are also available for telephone reassurance.
 
These services are provided to maximize independence and to enable clients to reside in the least restrictive environment, often in their own homes.
 
  • Why use a Care Manager?
If you have serious concerns about a frail or disabled adult and need information and assistance to ensure their safety, independence and quality of life, a care manager can be quite valuable.  If you have no family living nearby, then care management can assist you.
 
Care Managers do not replace family, but act as an extra member on whom you can rely on to advocate for your loved one.  We can help determine your loved one's needs, link them to the most appropriate services, and monitor them on an on-going basis. 
 
  •  What is the process?
Typically, an assessment is necessary for us to understand the environment the older adult or disabled person lives in, their level of functioning and their current support network.  The assessment involves an interview of the older or disabled adult and family members.  It may include the gathering of relevant medical records, the development of an individualized plan and the discussion of the options with the older or disabled adult and their family.
 
  • When is Care Management needed?
When an adult is faced with decreased capacity to function due to complications of aging, illness, disabililty, altered cognitive status and/or a loss of social support systems.  We can provide assistance when time and stress in providing care for a family member conflicts with work, family or other obligations.  Care management can be an essential link in the chain to assist family in caring for an adult who resides a long distance away.
 
Either short or long term support for clients and caregivers may be needed to assist in decision making and problem solving life issues such as: changes in physical or mental status, chronic illness, grief and loss, retirement, widowhood, relocation, or family conflict regarding how to best care for a family member.
 
  • How are Care Management services paid for? Billing fees and practices-
Most care managers provide private care management on a fee for service basis.  Fees are stated in written form and approved by the person accepting responsibility for payment prior to the start of services.  Services are not covered by Medicare, Medicaid or private insurance.  Care management fees are generally paid by the older or disabled adult, their estate, family members or other caregivers.
